1). Click on the Agreement Form link below to open the PDF file and SAVE, then PRINT and Complete Jack's Registration Letter of Agreement Form ( MUST bring the completed form to the first Class ).
2). Click on the West Chester Class Sign Up Form link to submit email reservation or you
can call either office to register by phone.
