Jack's Registration Letter of Agreement is displayed in Adobe Reader PDF format. If needed, a copy of a PDF file reader can be obtained at the following link.
1). Click on the Agreement Form link below to open the PDF file and SAVE, then PRINT and Complete Jack's Registration Letter of Agreement Form ( MUST bring the completed form to the first Class ).
2). Click on the Mason Class Sign Up Form link to submit email reservation or you
can call either office to register by phone.
